V02 Max - The Vital Health Metric That You Don’t Know

Most health-conscious people can rattle off their key numbers. Blood pressure. Cholesterol. Resting heart rate.

Ask them their VO2 max, though, and you'll usually get a blank stare.

That's a shame, because if you're someone who already pays attention to your health, this is one of the most valuable numbers you're missing.

What Is VO2 Max?

VO2 max is the maximum amount of oxygen your body can use during intense exercise. In simple terms, it's a measure of how efficiently your heart, lungs, blood vessels, and muscles work together to take in oxygen, deliver it around your body, and put it to use.

It's usually expressed as millilitres of oxygen used per kilogram of body weight per minute, and it's measured through an exercise test — typically walking, running, or cycling at increasing intensity while your oxygen consumption is tracked directly.

Why It's Different From Other Health Metrics

Here's what makes VO2 max stand apart from most of the numbers we're used to tracking.

Your blood pressure tells you about one system. Your cholesterol tells you about another. VO2 max is different — it's a measure of several critical systems working together under real physical demand:

  • Your lungs, taking in oxygen from the air

  • Your heart, pumping oxygenated blood out to the body

  • Your vascular network, delivering that blood efficiently to working muscle

  • Your muscles, extracting and using the oxygen once it arrives

Rather than isolating one system, it reflects how well the whole chain performs together — which is a large part of why it's such a meaningful number.

What the Research Actually Shows

This isn't just a fitness-industry talking point. The research base behind VO2 max (usually studied under the broader term "cardiorespiratory fitness," of which VO2 max is the gold-standard, directly measured version) is substantial.

A 2024 review in the British Journal of Sports Medicine pooled findings from 26 systematic reviews, representing over 20.9 million observations across 199 individual studies. It found that cardiorespiratory fitness had the largest single risk reduction for all-cause mortality of any factor studied - comparing high fitness to low fitness cut the risk of death from any cause by more than half.

A 2009 meta-analysis published in JAMA, one of the field's most-cited papers, established a clear dose-response relationship: as fitness increases, mortality risk drops in a stepwise fashion, with a widely used clinical threshold at 7.9 METs separating "low fitness" from meaningfully lower-risk territory.

Perhaps most interesting for anyone who's ever felt like their weight was the only number that mattered: a 2025 systematic review of nearly 400,000 people found that fitness remained a strong, independent predictor of mortality risk regardless of BMI. Fit individuals carrying extra weight showed no significant increase in mortality risk compared to fit individuals at a normal weight. While unfit individuals, at any weight, had two to three times the mortality risk of their fit counterparts.

In other words: fitness was doing more of the protective work than the number on the scale.

It's worth being precise here - no single metric, including VO2 max, tells you everything about your health. But few numbers carry this much predictive weight relative to how little attention they get.

Why Improving It Matters

Understanding your VO2 max is useful. But the real value comes from what you do with that information. Because unlike your genetics or your age, VO2 max is highly trainable, and improving it has a direct, felt effect on daily life.

Think about a flight of stairs. For someone with a high VO2 max, it's nothing - barely worth noticing. For someone with a low VO2 max, that same flight of stairs can be a genuine negotiation: pausing at the bottom, unsure whether they'll make it comfortably to the top.

Same staircase. Completely different lived experience. That's what this number quietly governs, not just how you'll perform in a gym, but how physically demanding ordinary life feels, today and years from now.

That's ultimately why it's worth knowing your number: not as an abstract statistic, but as a practical, trainable measure of how your future is likely to feel.
